How do I share my PDF record using the Docket app?

Need to share your personal and/or family immunization record PDF(s) for back-to-school, day care, college, a new job, or something else? Follow these simple instructions.

1. Select the "PDF" button that appears next to your name on the Immunization Records screen.

2. Select "Download PDF"

   Tap "Download PDF" next to the immunization report you would like access. Please note that PDFs could take a few seconds to download.

3. Select "Share PDF"

   After your PDF download is complete, tap the "Share PDF" button to share your official record. You can share using OS-standard functionality. If you received a new shot, tap "Refresh."

Where is Docket ◦ supported?

Docket is available to residents of Alaska, D.C., Idaho, Maine, Minnesota, Missouri, New Jersey, North Dakota, Utah, and Wyoming. Coming soon to more states.
